We aim to provide care and support to families across the country. Our high-quality live-in care enables people to live in the place they love most, their own homes. We are CQC-regulated members, focusing on enabling independence and enriching lives.

Brain Injury Care
As a fast-growing home care and complex care provider, we understand how challenging a brain injury can be for you, your loved one, and your family. The transition from the hospital or rehabilitation setting to your home can be a stressful and anxious experience.
End Of Life Care
We know that it can be a difficult and emotional experience when being told a friend or loved one’s illness is terminal. To then have to seek compassionate, trustworthy, and efficient end-of-life care to help make their final days, weeks, and months as comfortable and dignified as possible can be a daunting task.
Personal Care
We are able to support adults in their own homes with a wide range of personal needs. We offer care and support to adults of any age who need some assistance to continue living at home safely and comfortably. Our personal care teams provide friendly and professional help for older people
Overnight Care
We know that everyone’s needs differ from one person to another which requires a dedicated approach to fulfil one’s requirements. Therefore, for some, the care does not end by the end of the working day but continues, overnight. We provide Overnight Care for those who need some extra help during the twilight hours.
Dementia
Dementia is a degenerative and life-limiting disease that affects brain functioning leading to memory loss, language troubles (such as how to use words correctly or trouble speaking), the way you think, mood swings, difficulties with daily activities and more. However, everyone affected by dementia might experience different effects.
Elderly Care
We all grow old, it’s a fact of life – but this doesn’t mean we need to stop enjoying doing the things we love. With relatives leading busy working lives they can struggle to find the time to provide the care that matches their parents’ and grandparents’ needs. The next obvious choice may be care homes, but do you or your loved ones really want to leave .

HOW WE WORK

The Amazing Steps Of Our SUPPORT

1

Outline Your Needs

Centrestone Care Team receives your enquiry. We will discuss your requirements, and provide information on tailored services, that we can offer to meet the needs identified, as well as an outline of costs.

2

Assessor Home Visit

We will arrange for a Centrestone Assessor to visit your home and undertake a care and risk assessment. Following this visit, a fully costed care package will be offered.

3

Finalise Care Package

We will finalise your care plan with you and your family, and then identify the most suited care staff for you. We will also arrange for you to meet them, to ensure all your needs are met. If any additional training is required this will also be coordinated.

4

Service Commences

Your service commences. We will develop a comprehensive person-centred plan of care based on the identified needs. Your care plan is regularly reviewed to ensure it is suitable, for your health and social care needs.

why choose us

Compassion Is At The Heart Of Our Care

Your home is your security, whereby you can appreciate living with familiarity. Transferring to a residential home at such a sensitive time in your life can be associated with disruption and disorder, due to the changes, which can be deeply troublesome to adapt to.
